| A complete and certified set of finger prints from any local Police office after making a request |
| A copy of the first 3 pages of International Passport, which shows full name, place and date of birth, nationality, passport number, date and place of issue |
| Exact address(s) and dates of residence in Nigeria |
| Copy of the page of passport showing Nigerian Visa (if a non-citizen) |
| Full application and required documents to be forward in person or through any family member or friend to the office of: |
